Excessive side to side play in the axle, a "grating or grinding sound" if you rotate the axle without the chain on, binding or excessive slop or play.

How do i know if i need a new bearing carrier?

damn that carrier sold for cheap!
If you have this problem you need to remove your axle, then remove the old bearings inside the carrier and replace them with new ones. If you heat up the carrier in a over it usually helps them drop right out. Put the new ones in the freezer and they should drop right in.
